If you are not an ISA Member and would like to apply, submit your application today by visiting ASA’s website (link below) for the Membership Application form. You may also FAX your completed application to the ISA at (208) 344-7903. An invoice will be mailed to you for your ISA and ASA Annual Dues from ASA.

Active Military/Government Employee – $100

Affiliate $150

Medical Students $0

Educational (CRNA, Anes Tech, Veterinarian, Dentist) – $75
